The Continuing Care Division of Catholic Health Services of Long Island has consolidated its administrative, finance, home care and hospice offices into a single 49,000 square foot location on Bi-County Blvd. in Farmingdale. "This consolidation allows us to save approximately $1.3 Million per year in duplicative overhead and programmatic redundancies," points out Joseph Tomaino, Executive Vice President and CEO of the division. Clinical programs operating from the new site include Catholic Home Care, Good Shepherd Hospice, Good Samaritan Long Term Home Health Care program, and Our Lady of Consolation Long Term Home Health Care program, all under the direction of Mary Ellen Polit, Chief Administrative Officer. The new facilities make optimal use of technology, with a SmartHome display in the lobby to show patients and staff how advances can help maintain chronically ill at home, special tele-monitoring workstations, as well as state of the art conference and training facilities. One of the conference rooms has been set up to function as an Emergency Command Center in the event of a natural or human disaster. The design of the offices by CHS' Jim Proce and his team focused heavily on providing a positive and healthy work environment for staff, and several exam rooms were included in the human resources area so a nurse practitioner could upgrade the level of services provided by employee health. From this Continuing Care Center, care is coordinated and managed for over 2000 home care patients, nearly 400 hospice patients, about 790 nursing home residents, and over 1000 individuals with developmental or mental health disabilities each day.
